Description:

UMA Health is hiring Licensed Practical Nurses (LPNs) for home health visits in Suffolk, Norfolk, and Plymouth Counties. We offer flexible schedules, work/life balance, and one-on-one care. If you are interested in joining our team, please read on!

Responsibilities:

– Follow a care plan for each patient based on their individual needs.
– Visit assigned clients at their homes.
– Administer medication, wound care, and treatments as necessary.
– Monitor patients and report any changes in their condition to the Clinical or Patient Services Manager.
– Execute and document assigned tasks.
– Accurately document observations, interventions, and evaluations in our Electronic Health Record.

Qualifications:

– Must hold a current license as a licensed practical nurse in MA.
– At least one year of recent, verifiable clinical experience.
– Home care experience preferred, but not required.
– Graduation from an accredited LPN program.
– Ability to follow care plans and manage time effectively.
– Strong interpersonal skills.
– Solid computer skills; prior experience with electronic health records (EHR) preferred.
– Must be able to travel to assigned cases.
