What are Medicare-certified home health services?

Physicians prescribe short-term home health care services to aid in rehabilitation after an injury, illness, hospital stay, or surgery, or to manage a chronic medical condition to prevent unnecessary hospitalization. Specialized clinicians provide one-hour home visits until recovery goals are achieved. Typically, these episodes of care last between 30 and 60 days, but can be extended if required for medical reasons. Medicare and private insurance generally cover 100 percent of home health care visits through the Medicare home health benefit.

Your RN clinical manager oversees and manages your care plan and caregivers to ensure that all your care needs are met. They also communicate with your doctor as needed to make sure that you receive coordinated care. In addition, your client services manager plays a crucial role in coordinating your care schedule and is always available to answer any questions you might have. Together, our team is committed to providing you with the highest level of care and support.
